1. Fill out the form below with the restaurant's information & your information (very important).
2. After we receive your form submission we will contact the restaurant and try to sign them to njmenus.com.
3. If the restaurant you submitted signs up with NJ Menus, we will add that restaurant to your certificate account. Once you have 5 restaurants in your certificate account, we will personally buy you a $50 gift certificate to any restaurant in New Jersey of your choice.

In order to be awarded the free $50 gift certificate, the following must apply:
     • You must have 5 restaurants that you submit sign up with NJ Menus.
     • You must be the first person to submit that restaurant using the NJ Menus form.

Note: IIf you submit a restaurant, you will be notified by email whether or not it has been previously submitted. We will also contact you after we talk with the restaurant to let you know if they are signing up with NJ Menus.
